Having worked on indexes with similar name changes, it seems that a  
description page that gives all the working titles and the dates they were used  
works as long as there is no overlap.  The overlap can happen when one  paper 
absorbs another, in that case it is important to differentiate between the  
titles.
 
As for the John Does, as long as the index gives a date they might  provide 
information to someone who is looking for a missing person/death in an  
approximate period of time.  I always include them.  We have tended to  index 
them with no Surname and "unknown male/female" as the given name to have  
them sort out of the normal range of the index.  Again, a note on the  
description page can direct patrons where to look.
