[Genealib] Citation sheet

The "Reference Family Data Sheet" was from a workbook that went along with
"Tracing your ancestry" by F. Wilbur Helmbold. The workbook was sepatate and
had many helpful forms including family group sheets, pedigree charts,
census extraction forms, an outline map of the USA and a graph paper type
chart that was handy for charting what you wanted to look for and for whom.
It was a very useful book and I still have mine.
