[Genealib] Citation sheet

[Genealogy]

A short time ago someone on this list was looking for a citation sheet 
to place in a toploading poly notebook sleeve.  It was described as 
having alternating gray and white stripes and including spaces to 
completely describe the source of the resource that would be placed in 
the same sleeve.   In sorting through files today, I think I may have 
found a copy of this sheet.  The title at the top is Reference Family 
Data Sheet.  Along the left border is
