[Genealib] research request submission web form/ tracking system

Hello everyone,

The Survivors Registry is looking into using a research request
submission web form that would automatically put the information into
a tracking system/database.  We need a system that will let us:

1. automatically create a unique tracking number for each request
2. let us track who answered a request and when it was sent out
3. (preferably) archive a request once its been answered

Does anyone have a system like this, or know of request web forms that
you think are outstanding?  I would be interested in any stories about
creating a form like this, such as pitfalls to avoid.

If there is interest, I will send the results of what I find out.
